Piriformis Stretch
Three piriformis stretches, the hip angle that decides whether you reach the muscle, and how to tell piriformis pain from a disc problem before you stretch it.
How to do it
- 01Lie on your back with both knees bent and feet flat on the floor.
- 02Cross your right ankle over your left thigh, just above the knee, so the right shin makes a figure four.
- 03Reach through the gap and hold behind your left thigh.
- 04Draw the left thigh toward your chest until you feel a stretch deep in the right buttock — not in the front of the hip.
- 05Keep your head and shoulders relaxed on the floor. Hold 45 seconds, then switch sides.
The piriformis is a small muscle running from the sacrum to the top of the femur, deep beneath the glutes. It matters out of all proportion to its size because the sciatic nerve passes directly underneath it — and in a minority of people, straight through it.
That anatomy is why this stretch is worth doing carefully rather than enthusiastically.
Getting the angle right
The piriformis externally rotates the hip when the leg is down, but as the hip flexes past about 60 degrees its line of pull changes and it becomes an internal rotator instead.
Practically, that means the figure-four position — hip flexed, knee crossed over, leg rotated outward — puts it on stretch, and small changes in how far you draw the thigh toward your chest change which fibres you reach. If you feel nothing, adjust the angle of the crossed shin rather than pulling harder.
You should feel a deep, broad ache in the middle of the buttock. Sensation at the front of the hip means the crossed leg has rotated too far and you are pinching the hip joint instead.
Before you stretch it at all
Buttock pain is one of the most over-attributed symptoms in fitness. It gets blamed on the piriformis routinely when the source is the lumbar spine, the sacroiliac joint, or the hamstring tendon at its origin.
The distinction that matters most: does it travel? A deep ache confined to the buttock, worse after a long drive or a day at a desk, fits the piriformis story well. Sharp or electrical pain running below the knee — particularly with numbness, tingling, or weakness — points at nerve root compression higher up, and stretching a compressed nerve harder is the wrong response.
If it is the second, get it assessed rather than working through a stretching routine for six weeks.
Why it keeps coming back
The piriformis rarely tightens on its own. It is usually working overtime because the larger glutes are not doing their share, so it ends up stabilising a hip it was never built to stabilise.
Stretching gives relief that lasts hours. Strengthening the glutes gives relief that lasts. Use the stretch for symptom management, and treat weak hip abduction as the actual problem to solve.
Common questions
What is the best piriformis stretch?
The supine figure-four is the best default. Lying down removes the spine from the equation, the position is easy to control, and you can regulate intensity precisely by how far you pull the supporting thigh toward your chest.
How do I know if it is my piriformis or sciatica?
Piriformis-related pain is usually a deep ache in the buttock that worsens with prolonged sitting and improves with walking. True nerve root compression more often produces sharp, electrical pain travelling below the knee, sometimes with numbness or weakness. The second needs assessment, not stretching.
How often should I stretch my piriformis?
Once or twice a day, 45 seconds per side, is reasonable while symptoms are active. If two weeks of consistent stretching changes nothing, stretching is not the answer for your case and something else is driving it.
Why does the piriformis stretch hurt more on one side?
Asymmetry here is extremely common and usually reflects how you sit — wallet in one back pocket, legs crossed the same way, driving with one leg extended. The side that hurts is normally the side under more sustained load, not a structurally different muscle.
Can sitting too much cause piriformis pain?
Prolonged sitting keeps the muscle in a shortened, compressed position with your body weight pressing it against the sciatic nerve. It is the single most common aggravating factor, and standing up every half hour usually does more than any stretch.
